- Original Latest PSA Marriage Certificate *if married
- If your husband/wife is deceased, kindly provide Death Certificate
- Annulment/Divorced papers (if applicable)
- Photocopy of Valid and/or Government ID (back-to-back)
- Original Certificate of Employment (must be signed by the HR or any supervisor, indicate applicant's position, date hired, compensation, office address, HR landline number, and email address)
- Original Approved Leave of Absence (the date in your LOA must correspond to your travel date)
- Photocopy of Company ID
- Photocopy of Latest ITR (if no ITR, kindly provide a notarized affidavit explaining why there is no ITR)
- Photocopy of PRC Card or IBP Card (if applicable)
- Photocopy of DTI or SEC Permit (if under SEC, must include GIS, Articles of Incorporation, and By-Laws)
- Photocopy of Business Permit or Mayor's Permit
- Photocopy of BIR Certificate of Registration
- Photocopy of Latest ITR (if no ITR, kindly provide a notarized affidavit explaining why there is no ITR)
- Photocopy of PRC Card or IBP Card (if applicable)
- Notarized Affidavit of Support
- Proof of Funds (please refer to the list of documents for proof of funds)
- Proof of Relationship (Marriage Certificate)
